The Fort McDermitt Wellness Center does not currently have a medical provider for the following days:

Monday, November 26, 2018 through Friday, November 30, 2018

Patients can seek medical care at the Humboldt General Hospital – Emergency Room, so long as the following procedures are followed:

Upon return from ER visit, contact Sonya Hinkey, Patient Registration @ (775) 532-8522 – Option 1, and include the date and purpose of your visit.

Present your Medicare and/or Medicaid card or proof of private insurance, along with a state/federal issued identification card.

If you require non-emergency transport, please call Transportation to set up a ride.  Please give 36-hours notice if you want a ride, this will assure that we can can secure transportation.

The medication refill process has not changed.  All patients are urged to contact the Elko Service Unit Pharmacy located at the Southern Bands Health Clinic at (775) 532-1437 to refill prescription medications.

You will be required to provide your medical record (chart number).  If you do not have this number, contact Sonya Hinkey or Taela Tobey by calling the Wellness Center’s Patient Registration at (775) 532-8522 – Option 1.
