Marketplace and Medicare Insurance Enrollment

You are invited to come to this enrollment event. If you have had questions regarding your Medicare enrollment and want answers make sure to stop by and have those questions answered. Certified Medicare counselors will be present to assist you as needed. If you plan on making changes you will need to bring the following:

• Medicare Card
• Current Part D Card
• List of Medications

Do you not qualify for Medicaid, but need/want insurance coverage? Is the cost of insurance coverage a hindrance to you and your family? The Fort McDermitt Wellness Center now has sponsored insurance for the tribal members of the Ft. McDermitt Paiute-Shoshone Tribe. This means tribal members can now have insurance without the concern of covering the costs of their insurance for themselves and their family. • Photo ID
• Proof of Income – last 3 paycheck stubs
• Social Security Cards – for all members needing insurance
• Tribal ID Card
